Someone told Claude "make it better" 100 times in a row. By round 60 it was breaking its own website.
Same prompt, two models. Sonnet 5.5 made small changes all the way through. Opus 5.5 kept adding features until the site hit 21,000 lines of code and started breaking things it had built earlier.
I build these for clients, and I've had my own smaller version of this.
I told a research agent to find more prospects for a campaign. Its searches came back thin, so it invented six people. One had a LinkedIn URL that doesn't exist. It admitted this in its own summary, which is the only reason I caught it.
Nobody gave it a stopping condition, so it kept going.
Before any agent step goes live now, I want one thing written down: how it knows it's done.
For that agent it meant a rule that zero results is an acceptable answer, plus four random leads checked by hand before any list goes out. Anything touching money or a customer still waits for a human.
Next time someone pitches you an AI agent, ask what it does when it can't find what it's looking for. The demo won't show you that part.
